Cancer2FinishLine (C2FL) Application Process

I’m so excited that you are interested in applying for the Cancer2FinishLine Program! In order to be considered for the program, please follow the instructions below:

1. Print the Oncology Authorization Form and have your oncologist complete and sign the form.
2. Scan the form to prepare to upload it into the C2FL Application form.
3. Complete the Cancer2FinishLine Program Application
